Organization Representative Pre-Test Survey  

  “Toward Positive Change to Promote Mental Health and Wellbeing for Black Canadians in Manitoba” 

Thank you for taking the time to complete the survey. Your feedback is very important. The purpose of the survey is to assess the extent to which the Mental Health Promotion Toolkit for Black Canadians is effective in promoting mental health awareness for Black Canadians in Manitoba. Please note that completing this survey is voluntary. The survey is divided into two sections. The first section contains individual-level questions and the second section contains questions regarding the organization you are representing. The results of the survey will be collated and reported in aggregate. Please be assured that all information collected in this survey is strictly confidential. The survey takes up to approximately 15 minutes to complete.
